Team captains: Collect money from your team members. This form will sign up YOUR WHOLE TEAM in one transaction.

If you don’t have a foursome, you can register with 3, 2 or even 1 player. We’ll pair you up with another non-foursome.

Ask your teammates for their T-shirt sizes. Don’t guess their preferred size. We won’t be able to change shirt sizes after you register. You can also preorder more shirts here on the site. We’ll have a limited number of assorted sizes for sale at Royal Docks after the golf scramble.

Remember, all proceeds go to Hope United to help fight the opioid crisis in our area. So buy some DGBD19 shirts for your friends and family!
